Please contact your school nurse for questions about acute and chronic illness, acute injury, concussion, planned or unplanned surgery, prescription and over the counter (OTC) medications at school, or any health concerns. Your school nurse will work with building administrators and school staff to support your student with short term adjustments while your student recovers from an acute illness, injury, or concussion. Your school nurse will also help your student transition to long term accommodations if needed.

Form Completion:
- District Medication Policy JLCD
- Medication Permission Slip: expires every school year
- Medication Self Carry Contract: expires every school year
Individual Health Care Plans: Expires one year from physician’s signature/date. If your student self carries an emergency medication or the health office administers an emergency medication your student’s medical provider needs to fill out one of these forms and the Self Carry Contract*:
